How to make a SportsWare account

 

    1. Go to  www.swol123.net. 
    2. On the right-hand side there is a box that says “Athlete/Parent” click the blue box that says “Join SportsWear”
    3. The school ID is LCHurricanes
    4. Fill out ALL the information boxes
    5. It will then go to the Athletic Trainer for approval (we check SportWare at the start and end of every work day)
    6. Once approved login with your email and password and click forms and complete ALL of the forms listed below
      1. Athlete Packet
      2. My Info (all areas with red asterisk)
      3. Address
      4. Emergency Contact
      5. INSURANCE
      6. Medical- under alerts be sure to include ANY allergies
    7. Click save once all the information has been entered
    8. Click MED HISTORY
      1. Select your sport(s)
      1. Answer yes or no to the Medical History
      2. Click save
    2. Once completed it should say 0 forms left to complete/download on your main page

The ENTIRE profile MUST be completed before you will be cleared for any athletic participation
